Remote Patient Monitoring Programme

1. The questionnaire is sent by e-mail to the patient 1, 3, 6 months and annually after the operation. 2. The results of the questionnaire allow the cardiologist to assess the patient's condition, if necessary, to adjust recommendations for drug therapy and timely prescribe additional examination in case of deterioration of well-being. 3. If any alarming factors are detected, the cardiologist will contact the patient and give all necessary recommendations. To participate in the Programme you need to inform the employee of the Centre about the e-mail address where the questionnaire will be sent and give your agreement to participate in the paid Follow-up Programme for the first 3 months after the operation and 6 months. The first consultation is obligatory

in a personal format (by referral of the attending physician or on a paid basis), where you will receive all the necessary information, additional materials for further follow-up. Work direction The clinical and analytical department of the Centre works only with operated patients to ensure: Corrections • warfarin dose under INR control in order to achieve target values; • therapy prescribed at the Centre, physical activity, lifestyle in the early postoperative period. Consultations • in case of problems with the postoperative wound; • preparation for a second surgical intervention (if necessary); • dynamic monitoring to avoid possible complications.
