Specialized medical care at the Center is provided free of charge in inpatient and day hospital conditions by specialist doctors and includes the prevention, diagnosis and treatment of diseases and conditions that require the use of special methods and complex medical technologies.

Medical assistance in a round-the-clock or day hospital is provided on the basis of clinical recommendations, taking into account the procedures and standards for the provision of medical care.

Referral of a patient for planned hospitalization to a round-the-clock or day hospital is carried out by the attending physician of the outpatient department of a medical organization in accordance with clinical indications.

The choice of a federal medical organization for receiving SMP in a planned form is carried out in the direction of the attending physician.

The attending physician is obliged to inform the patient/legal representative of the patient about the possibility of choosing a federal medical organization, including the possible waiting times for specialized medical care, which may exceed the waiting times established by the program of state guarantees of free medical care for citizens.

For planned hospitalization, it is necessary to have a referral issued by the attending physician containing data from an objective examination, the results of additional laboratory and instrumental studies.

When issuing a referral for the provision of specialized medical care in a planned form to a round-the-clock or day hospital, the attending physician is obliged to inform the citizen about the medical organizations participating in the implementation of the Program, in which it is possible to provide appropriate medical care, taking into account the waiting time. Based on the information received, the citizen chooses a medical organization.

If a citizen chooses a medical organization in which the waiting period for specialized medical care exceeds the waiting period established in the Program, the attending physician makes a corresponding note in the medical documentation.

Indications for referral of a patient to the Center (paragraph 7 of the Procedure for referral of insured persons to medical organizations, approved by Order of the Ministry of Health of Russia dated December 23, 2020 No. 1363n):

• Atypical course of the disease and (or) lack of effect from the treatment;
• The need to use treatment methods that are not performed in medical organizations operating in the field of compulsory health insurance within the framework of the territorial program of compulsory health insurance;
• High risk of surgical treatment due to the complicated course of the underlying disease or the presence of comorbid diseases;
• The need to perform repeated surgical interventions in the cases provided for by the first three paragraphs;
• The need for additional examination in diagnostically difficult cases and (or) in cases of complex preoperative preparation in patients with complicated forms of the disease and (or) comorbid diseases for subsequent treatment;
• The need for re-hospitalization on the recommendation of the Center or other federal medical organization.

The basis for the hospitalization of a patient in the Center is the decision of the Commission of the Center for the selection of patients for the provision of specialized, including high-tech care (hereinafter referred to as the Commission of the Center) (Order of the Ministry of Health of Russia dated December 23, 2020 No. and the powers of the founders in respect of which are exercised by the Government of the Russian Federation or federal executive bodies to provide medical care in accordance with the uniform requirements of the basic program of compulsory medical insurance "(hereinafter - Order of the Ministry of Health of Russia dated December 23, 2020 No. 1363n)) on the presence of medical indications for hospitalization taken on the basis of the provided medical documents.

The patient (his legal representative) has the right to independently submit a completed set of documents to the Commission of the Center for making a decision on hospitalization.

The list of documents that are necessary for making a decision on hospitalization by the Commission of the Center (paragraphs "a" - "c" of paragraph 9 of the Order of the Ministry of Health of Russia dated December 23, 2020 No. 1363n):

The patient himself can provide the necessary list to the Center.

a) Referral for hospitalization, examination, consultation on the form 057 / y-04:

• surname, name, patronymic (if any) of the patient;
• date of birth, patient, address of registration at the place of residence (stay);
• the number of the compulsory medical insurance policy and the name of the insurance medical organization (if any).
• (All newborn citizens from December 1, 2022 will be automatically entered into the register of insured persons on the basis of a birth certificate issued by the registry office.)
• insurance certificate of compulsory pension insurance (SNILS) (if any);
• code of the main diagnosis in accordance with the International Statistical Classification of Diseases and Related Health Problems, tenth revision (hereinafter - ICD-10);
• the results of laboratory, instrumental and other types of studies confirming the established diagnosis and the presence of medical indications for the provision of specialized medical care;
• the profile of the specialized medical care shown to the patient and the conditions for its provision (inpatient, day hospital);
• the name of the medical organization to which the patient is sent to provide specialized medical care;
• surname, name, patronymic (if any) and position of the attending physician, contact phone number (if any), email address (if any).
• When forming a referral on paper, it must be written legibly or in printed form, certified by the personal signature of the attending physician, the personal signature of the head of the referring medical organization (authorized person), the seal of the sending medical organization.
• When forming a referral in the form of an electronic document, it is signed with enhanced qualified electronic signatures of the attending physician, head of the referring medical organization (authorized person).

b) Extract from medical records, which must contain:

• diagnosis of a disease (condition),
• ICD-10 diagnosis code,
• information about the state of health, carried out diagnostic tests and treatment,
• recommendations on the need for specialized medical care;

c) Consent to the processing of personal data of the patient / legal representative.

Waiting periods for medical care provided in a planned manner (established by the territorial program of state guarantees of free provision of medical care to citizens):

14 working days from the date of issuance by the attending physician of a referral for hospitalization for the provision of specialized (except for high-tech) medical care;

Types of high-tech medical care 2023.

High-tech medical care is part of specialized medical care and includes the use of new complex and (or) unique methods of treatment, as well as resource-intensive methods of treatment with scientifically proven effectiveness, including cell technologies, robotic technology, information technology and genetic engineering methods developed by based on the achievements of medical science and related branches of science and technology.

VMP is provided by medical organizations in accordance with the list of types of high-tech medical care approved by the authorized federal executive body. (Decree of the Government of the Russian Federation of December 28, 2021 N 2505 "On the Program of State Guarantees of Free Medical Care for Citizens for 2022 and for the planning period of 2023 and 2024"

The direction of the patient and medical indications for the provision of high-tech medical care is determined by the attending physician of the medical organization in which the patient is being diagnosed and treated as part of the provision of primary specialized health care and (or) specialized medical care, taking into account the right to choose a medical organization.

The basis for the hospitalization of a patient in the Center is the decision of the Commission of the Center. If a decision is made to provide HTMC, the Center issues a Voucher for HTTC for the patient using a subsystem of a single system with a set of documents attached:

• an extract from the medical documentation containing the diagnosis of the disease (condition), the ICD diagnosis code, information about the patient's health status, the results of laboratory, instrumental and other types of studies confirming the established diagnosis and the need for VMP;
• document proving the identity of the patient;
• the patient's birth certificate (for children under the age of 14);
• compulsory health insurance policy. (All newborn citizens from December 1, 2022 will be automatically entered into the register of insured persons on the basis of a birth certificate issued by the registry office.)
• compulsory pension insurance certificate
• consent to the processing of personal data of the patient and (or) his legal representative;
• application for review of documents. To be filled in by the patient/legal representative according to the form, with the obligatory completion of all columns. The application form can be found on the website astra-cardio.ru, at the information kiosk, at information stands.

The patient (his legal representative) has the right to independently submit the completed set of documents to the Center, in case of provision of HTMC included in the basic program of compulsory medical insurance.

The result of consideration of documents is documented in the Protocol of the Commission.

Documents can be submitted via: self-appeal of the patient or his legal representative, directly to the Center on business days from 09.00 -12.00.

The term for consideration of documents is 10 working days.

The time of issue of the Protocols of the Commission - working days from 15.00-16.00

The period of storage of documents on paper in the Center is 30 working days.

To provide high medical care at the expense of funds provided from the federal budget, the direction of citizens of the Russian Federation is carried out by using a specialized information system and in accordance with the procedure established by the authorized federal executive body in accordance with the Order of the Ministry of Health of Russia dated 02.10.2019. 824n "On approval of the procedure for organizing the provision of high-tech assistance using a unified state information system in the field of healthcare").
