Medical assistance is provided in the following forms:

Medical care for conditions that do not require round-the-clock medical supervision and treatment for medical reasons is provided to citizens on an outpatient basis, including on day hospital beds in conditions that provide for medical supervision during the day, but do not require round-the-clock medical supervision and treatment.

In conditions requiring, for medical reasons, round-the-clock medical supervision and treatment, medical care is provided in a round-the-clock hospital.

Primary health care in the Center is provided on an outpatient basis, in planned and emergency forms.

Primary health care, included in the Territorial Program of State Guarantees of Free Provision of Medical Care to Citizens (hereinafter referred to as the Program), is provided on an outpatient basis:

Primary specialized health care on an outpatient basis is provided only on the referral of the attending physician (in the absence of a referral, the patient has the choice to receive the necessary assistance on a paid basis).

Indications for referral to a consultation appointment with a doctor - a specialist of the Center:

The list of documents that the patient must have when contacting the Center:

  1. Passport or Birth Certificate (children under 14).
  2. Compulsory health insurance policy. (All newborn citizens from December 1, 2022 will be automatically entered into the register of insured persons on the basis of a birth certificate issued by the registry office.)
  3. Certificate of compulsory pension insurance (SNILS) - original
  4. Certificate of establishment of a disability group (if there is a disability group)
  5. An extract from the medical card of an outpatient (inpatient) patient.
  6. Results of diagnostic studies (if available)
  7. Referral for examination, consultation in the form 057 / y-04 to the Federal State Budgetary Institution "FTSSSH" of the Ministry of Health of Russia (Astrakhan) with a mandatory note about the purpose of the referral.

The referral must be drawn up in accordance with regulatory documents indicating the purpose of the visit and contain the results of a preliminary study in accordance with the profile of medical care.

Laboratory and instrumental research methods are provided to citizens in the direction of the attending physician if there are medical indications, taking into account the procedures and standards for the provision of medical care.

Waiting periods for primary specialized health care provided in a planned manner (established by the territorial program of state guarantees of free provision of medical care to citizens):

Adult: Monday - Friday from 8:00 to 19:00

Children: Monday - Friday: from 8:00 a.m. to 4:00 p.m.

When applying for a consultation again, patients must have a referral for a consultation in the form 057 / y-04 from the attending physician and a brief extract on the treatment performed.
